Senior Product Manager, Care Operations

at Zócalo Health

Remote (Full Time)

Compensation: $150,000/ year

About Us

Zócalo Health is a tech-enabled, community-oriented primary care organization serving people who have historically been underserved by the one-size-fits-all healthcare system. We partner with health plans, providers, and community organizations to deliver culturally competent primary care, behavioral health, and social care.

Our model is built for populations with high medical and social complexity, where fragmented care drives poor outcomes and unnecessary cost. We combine local, community-based teams with virtual care and modern technology to deliver coordinated, whole-person care where members live and receive support.

Founded in 2021, Zócalo Health is backed by leading healthcare and mission-aligned investors and is scaling rapidly across states and populations. We are building a durable care platform designed to perform in constrained healthcare environments and to lead the shift toward accountable, value-based care.

Role Description

The Senior Product Manager, Care Operations will join Zócalo Health at a critical stage of the company’s growth, as we expand our care model across programs, plans, and geographies.

This role owns the Care Operations product domain end-to-end. You will shape how our care teams do their work by defining and driving the roadmap for mission-critical workflows, partnering closely with Engineering, Operations, and Clinical teams. You will collaborate with the VP of Product on strategy, sequencing, and major tradeoffs, while operating autonomously within your domain.

This role carries dual responsibility: stewardship of the systems care teams depend on today, and building the workflows they need as Zócalo scales. You will own the health and evolution of existing operational tools while driving the transition to governed, instrumented workflows that replace fragmented processes.

This role is intentionally broad at the outset and evolves as the Product team scales. As additional Product Managers join, this role may shift toward systems migration ownership, cross-team operational consistency, and vendor transition management — high-leverage work that directly supports Zócalo’s ability to scale without accumulating operational debt.

The systems you build will directly influence how care is delivered to patients who have historically been underserved by the healthcare system, shaping both product outcomes and the day-to-day experience of our care teams as Zócalo scales its care model.

This position reports to the VP of Product.

The Senior Product Manager, Care Operations will contribute in the following ways:

  • Own the Care Operations product roadmap end-to-end, including problem identification, prioritization, and delivery
  • Own product direction for outreach, intake, authorization, scheduling, and care team tooling
  • Define and drive your own roadmap, grounded in real care team needs and aligned with company priorities, rather than executing against a pre-defined backlog
  • Partner day-to-day with Engineering, external development partners, and Design to ship, iterate, and improve workflows
  • Establish direct feedback loops with care teams, including CHWs, Operations, and Clinical staff
  • Define requirements, acceptance criteria, and success metrics for shipped features
  • Ensure new tools are adopted, usable, and meaningfully improve how care teams work
  • Own stewardship of existing care operations systems, ensuring they are reliable, usable, and not creating unnecessary operational or financial drag
  • Partner with Operations, Clinical, and Finance to determine when to improve, tolerate, or sunset existing tools based on real usage and impact
  • Own system transition and sunset sequencing for legacy tools within the Care Operations domain, including defining readiness gates and cutover plans

Qualifications

  • 5+ years of product management experience or equivalent experience owning complex operational systems
  • Experience working on healthcare operations, care coordination, or EHR-adjacent products
  • Experience managing system transitions, migrations, or vendor sunsets
  • Comfort balancing stewardship of existing systems with forward-looking product development
  • Strong ability to operate in ambiguous, early-stage environments
  • Demonstrated ownership of outcomes, not just delivery of requirements
  • Experience partnering closely with Engineering, Design, and Operations teams
  • Excellent communication and stakeholder management skills
  • Comfort using AI-enabled tooling to accelerate product development and operational workflows

Preferred Qualifications

  • Experience with care delivery workflows, value-based care, or Medicaid populations
  • Familiarity with Athena or similar EHR systems
  • Experience working with external development partners or vendors
  • Background in workflow-heavy or operationally complex domains
  • Experience operating across multiple product teams, maintaining consistency and standards while teams execute independently
