Role Description
The Head of Community Supports will lead the design, implementation, and oversight of Zócalo Health’s Community Supports (CS) initiatives as part of the Cal AIM Enhanced Care Management benefit. Reporting to the VP of Clinical Operations, this role will focus on scaling CS services to meet member needs, ensuring compliance with regulatory standards, and driving impactful health outcomes. The ideal candidate is a strategic leader with a deep understanding of community health programs and a commitment to health equity.
About the Role

Program Leadership and Development

Design and implement Community Supports services, including Housing Navigation, Housing Tenancy, and Day Habilitation.
Develop workflows, policies, and procedures to guide CS operations and service delivery.
Lead county-based implementation strategies, ensuring scalability and sustainability.
Collaborate with the Head of Payer Contracting and Strategy with the contracting, coordination, onboarding and management of ECM/CS network and programs.
Support operational compliance and reporting to satisfy and exceed state and federal standards.

Team Management

Build and manage the CS team, including county-based leads.
Establish and maintain a caseload ratio of 1:45, ensuring optimal service delivery.
Provide regular coaching, supervision, and training to the CS team to maintain high standards of care.

Operational Oversight

Collaborate with cross-functional teams to develop the infrastructure needed for CS program success.
Monitor performance metrics, quality standards, and compliance requirements for all CS services.
Partner with the leadership team to align CS services with member needs and organizational goals and support the application and contracting process for expanding program reach.
Ensure that related policies and procedures necessary to operate the department comply with regulatory and accreditation requirements and are in place and updated no less than annually.

Community Engagement and Partnerships

Build and maintain relationships with community-based organizations, county agencies, and other key stakeholders particularly in the housing and homelessness sectors.
Represent Zócalo Health in external meetings, coalitions, and advocacy efforts to enhance the impact of CS programs.

Reporting and Data Management

Oversee the collection and analysis of program data to track progress, outcomes, and opportunities for improvement.
Generate reports for internal leadership and external stakeholders, including health plans and regulatory entities.
Requirements for the Role

Education

Bachelor’s degree in Public Health, Healthcare Administration, Social Work, or a related field. Master’s degree preferred.

Experience

5+ years of leadership experience in healthcare operations, community health, or social services programs.
2+ years of direct supervisory experience.
Proven experience with CalAIM initiatives including Enhanced Care Management and Community Supports programs or similar initiatives.
Strong knowledge of community health needs and social determinants of health.
Experience in delivery and implementation of large-scale health plan initiatives.
Knowledge of Health Plan Core functions of Medi-Cal and CalAIM
Knowledge of Health Plan/ Integrated Delivery system models of case management and care coordination best practices.
