Nurse Practitioner

In order to deepen our impact on people’s lives, our care will extend beyond the care provided by physicians within the confines of the exam room. We are introducing a long overdue improved experience for the BIPOC community that is built on trust, community, and a holistic approach that enables members to achieve optimal social, mental and physical well-being. Providers in our network recognize these inequities and treat our patients with empathy, clear information, and a solution that is cognizant of their background and needs. It is a tireless pursuit that will improve the lives of our patients and will build sustainable, healthier communities.

There is a need for an interdisciplinary effort which is why our care teams consist of promotoras / community health workers, physicians, health coaches, and patient advocates. We do this in an effort to help our community be healthier so they can live a more fulfilling and healthy life. We will first open in California with expansion into Washington and Texas.

The Virtual Clinic Nurse Practitioner works in collaboration with the supervising physician and additional members of the health care team to participate in the diagnosis, treatment, and management of virtual clinic patients. The Nurse Practitioner will incorporate both independent and collaborative decision-making in accordance with established clinical protocols and standardized procedures to provide high-quality evidence-based care to clinic patients. Responsibilities will include video visits and asynchronous communication with patients.
