About The Role
As a Patient Navigator at Season, you will support patient care and clinically-related customer service across a spectrum of health conditions and nutrition-related needs. You will provide collaborative, comprehensive and person-centered care and education to support behavior change and improved quality of life and advocate for patients as appropriate. You will work closely with experts across our organization as well as partner with clinicians and patients to support best-in-class outcomes, refine our platform, and hone our strategies for improving engagement. A key component of your role will also be to facilitate communication with providers. This role will work up to 40 hours per week Monday to Friday 9 am to 5 pm Pacific Time.
What You Will Do
- Uphold best practices in patient care.
- Build relationships with clinical customers to facilitate engagement and understand customer needs.
- Document all individual contacts/visits in the electronic health record and outcomes database according to guidelines and in a timely manner.
- Work with providers to communicate recommendations, when appropriate.
- Provide pre- and post-dietitian visit outreach to engage patients and assist in tracking outcomes.
- Actively participate in quality improvement processes.
- Collaborate with clinicians to work on interventions to meet quality metrics of patients.
- Contribute to the achievement of established clinical goals and objectives and adheres to department policies, procedures, quality standards and safety standards.
- Participate in meetings as appropriate.
About You
- Maintain RD or DTR credentialing with the Commission on Dietetic Registration, or other relevant credentialing, as appropriate, or a degree in a nutrition-related field
- Knowledge of food and current nutrition trends as well as best practices in nutrition care
- Eagerness to learn and discover new ideas, solve problems
- A track record of creating and deploying new evidence-based tools, and moving quickly
- A demonstrated ability to think strategically about clinical solutions as well as the hands on skills to solve customer issues
- Excited by the prospect of playing a foundational role and helping to establish Season’s product
- Passion for working in an early-stage company and building from the ground up
- Comfort with ambiguity
- Excellent written and oral communications skills
- Ability to inspire and collaborate with colleagues from a wide array of backgrounds
- An overdeveloped sense of ownership
- Highly organized and strong attention to detail
- Must be able to work with a diverse patient population and have exceptional customer service skills
Compensation and Benefits
- The rate for this role is $26-$30 per hour, dependent on qualifications.
