Job Description
We are seeking an experienced Care Coach & Licensed Social Worker to join our stellar Service team, operating as the tip of the spear in providing personalized coaching, guidance, support, and training to the family caregivers we work with. You should be caregiver-centered, detail oriented, resourceful, and driven to provide incredible support that makes a difference in people’s lives.
We are seeking an experienced Licensed Social Worker to join our team as a Care Coach, responsible for building caregiver efficacy and alleviating caregiver burden over time through proactive coaching, guidance, and consistent support.
This role is relationship-driven and longitudinal in nature. While acute or crisis situations may arise, the majority of impact comes from preventing escalation, reinforcing best practices, and helping caregivers build confidence, capability, and independence over time.
This role is best suited for individuals who find meaning in steady, preventative work, proactive guidance, and long-term impact, rather than constant clinical intensity or crisis-driven intervention.

Responsibilities
- Provide ongoing, longitudinal coaching to caregivers, developing trust, building confidence, reinforcing best practices, and reducing caregiver burnout over time.
- Monitor caregiver care notes and ADLS/IADLS task completion via the Givers mobile application, identifying patterns, risks, and opportunities for proactive support.
- Conduct scheduled monthly calls and manage incident triage as needed, including appropriate support, documentation, and escalation.
- Ensure excellence in documentation, compliance, and organization; contribute to improving how coaching work is delivered across systems and tools.
- Maintain consistent, timely communication with caregivers, nursing staff, and Medicaid waiver case managers to support continuity of care.
- Collaborate internally to improve service quality, consistency, and scalability through data-informed insights and process refinement.
- Take ownership of assigned projects that improve caregiver experience, team effectiveness, or service delivery operations.
Requirements
- Active LSW required, Ohio licensure preferred
- Minimum of 3-5 years of experience in Medicaid waiver programs, or similar fields, preferably working in Structured Family Caregiving with caregivers, elders or people with disabilities in regulated settings.
- Strong communication, relationship building, assessment, and problem-solving skills.
- Proficiency with case management systems and mobile applications.
- Ability to work independently and maintain effective communication channels with all stakeholders.
- Ability to navigate community resources related to medical, housing, financial, and social issues for clients' care.
- Compassionate, understanding, and able to handle stressful situations with empathy and professionalism.
- Strong organizational skills and meticulous attention to detail for compliance and regulatory standards.
- Proficient in Zoho, Zendesk (or other Customer Success Portals), Notion, Google Drive, Slack, and other software tools.
