Job Description
We are seeking an experienced Care Coach to join our stellar Service team, operating as the tip of the spear in providing personalized coaching, guidance, support, and training to the family caregivers we work with. You should be caregiver-centered, detail oriented, resourceful, and driven to provide incredible support that makes a difference in people’s lives.
This is a full-time remote (WFH) role, and you must live in the Eastern Time Zone of the US.
Responsibilities
- Provide ongoing coaching, support and guidance to caregivers, developing trust, building confidence and reinforcing best practices to influence change and reduce caregiver burnout.
- Monitor family caregivers' daily progress notes and tasks performed via the Givers mobile application.
- Conduct monthly supervisory calls and semi-annual in-person case conferences with clients, caregivers, RNs, and care coordinators.
- Ensure excellence in documentation & organization; adapting & improving practices across systems & tools.
- Maintain effective & timely communication with primary family caregivers, registered nurses (RNs), and Medicaid waiver case managers.
- Collaborate internally to proactively identify and implement data-driven opportunities to improve our service & systems as we grow.
- Other projects & duties as assigned.
Requirements
- Minimum of 3-4 years of experience in home care, Medicaid waiver programs, or similar fields, preferably working with elders or people with disabilities in regulated settings.
- Bachelor's degree in social work or an equivalent combination of education and experience in related fields.
- Strong communication, relationship building, assessment, and problem-solving skills.
- Proficiency with case management systems and mobile applications.
- Ability to work independently and maintain effective communication channels with all stakeholders.
- Ability to navigate community resources related to medical, housing, financial, and social issues for clients' care.
- Compassionate, understanding, and able to handle stressful situations with empathy and professionalism.
- Strong organizational skills and meticulous attention to detail for compliance and regulatory standards.
- Proficient in Zoho, Zendesk (or other Customer Success Portals), Google Drive, Slack, and other software tools.
This is a full time position with a salary of $60,000 per year. You will also be eligible for benefits including health, dental, and vision insurance, a 401(k) program, and more.
