Job Description

As a Care Coach, you will be the tip of the Givers spear–providing training, guidance, and customer to our family caregivers. You should be extremely efficient, organized, and resourceful.

Responsibilities

  • Conduct family-centered assessments and identify support needs for caregivers to help provide care at home.
  • Monitor family caregivers' completion of daily progress notes and tasks performed via the Givers mobile application, and reach out to those who did not submit their daily notes every morning.
  • Conduct monthly supervisory calls with clients, caregivers, RNs, and care coordinators.
  • Provide ongoing health coaching, training, and support to family caregivers to ensure client goals are met, responding to requests within 1 hour on weekdays and 4 hours on weekends.
  • Perform other duties as assigned.

Requirements

  • Experience with high volume case management or customer support.
  • High tech savvy, innate drive to respond quickly and document everything.
  • Minimum two (2) years of experience in home health, Medicaid waiver programs, or similar fields, preferably working with elders or people with disabilities.
  • Bachelor's degree in social work or an equivalent combination of education and experience in related fields.
  • Strong communication, relationship building, assessment, and problem-solving skills.
  • Ability to work independently and maintain effective communication channels with all stakeholders.
  • Ability to navigate community resources related to medical, housing, financial, and social issues for clients' care.
  • Compassionate, understanding, and able to handle stressful situations with empathy and professionalism.
  • Strong organizational skills and meticulous attention to detail for compliance and regulatory standards.
  • Proficient in Zoho, Zendesk (or other Customer Success Portals), Google Drive, and other software tools.
