Amae Health is seeking the right Clinical Care Coordinator to partner with us in providing care for complex adult patients. This position will be full-time, in person at our Houston clinic.
About the Job
The Clinical Care Coordinator will act as a coach and mentor and help patients to set goals and work toward developing skills. They will share and discuss experiences with patients. The peer will help to build a collective sense of community for patients and help patients to create meaningful lives in their respective community. The Clinical Care Coordinator will be responsible for working with the broader clinical team to plan, organize, develop, lead and facilitate group activities, including education and awareness building efforts. They will complete administrative duties, such as sending and receiving email and telephone calls, and completing required documentation. They will also complete any training required for the position. The candidate will communicate and work collaboratively with team members, attend team meetings, and meet with supervisors to discuss performance.
- Assist in the development and implementation of patient activities, such as games, arts and crafts, and other recreational activities
- Interact with patients to promote positive socialization and encourage participation in treatment activities
- Work closely with the treatment team, including psychiatrists, therapists, and nurses, to coordinate care and improve patient outcomes
- Help maintain patient records and update treatment plans as needed
- Participate in training sessions and other learning opportunities to improve skills and knowledge related to mental health treatment
- Participates in clinical meetings and discussions to review current and new patients in treatment
- Regularly assesses patients ongoing needs and provides the appropriate community resources
About You
- Enrolled or recently graduated in a degree program related to mental health, such as pre-med, psychology, social work, or nursing
- Interest in mental health and desire to work with patients with Severe Mental Illness
- Strong interpersonal skills and ability to work in a team environment
- Detail-oriented and organized
- Comfortable working either one-on-one or in group settings, must possess excellent interpersonal skills and be able to adapt to changing situations
- Willingness to collaborate with others
- Well-organized and some experience planning and designing events and activities
- Basic computer skills are required
- The candidate should possess a holistic perspective of “health.” Clinical Care Coordinators will demonstrate the ability to work effectively in a wide range of settings with people from diverse backgrounds, including patients and co-workers
