Amae Health is an integrated, outpatient care provider for patients with SMI, treating their psychiatric and physical conditions through in-person care delivery with wrap-around services. Amae Health is seeking a Clinical Care Coordinator to partner with us in caring for complex adult patients. This position will be full-time, in office.
About the Job
The Clinical Care Coordinator (CCC) plays an integral role in fostering a supportive and welcoming environment for our members. Acting as a coach and mentor, the CCC assists members with SMI in setting meaningful goals, developing life skills, and building a strong sense of community. This role involves managing administrative duties and coordinating and facilitating group activities.
● Front Desk & Member Support: Serve as the first point of contact for members, warmly greeting them upon arrival, orienting members, answering phones, addressing inquiries, and notifying staff of arrivals. Ensures all members feel welcome and provides support during challenging moments.
● Administrative Duties: Calls members who missed appointments, manages lab orders, tracks attendance, and assists with documentation and treatment plan updates. Works collaboratively with the clinical team to support scheduling and ensure seamless communication.
● Group Facilitation: Leads morning check-in groups, guiding members through journaling prompts and check-in exercises to start the day positively. Facilitates activity-based group sessions and engages members in meaningful discussions.
● Member Engagement & Intervention: Acts as a source of support for members throughout the day. Recognizes and responds to members’ emotional needs with empathy and composure, communicating with the broader team when a member may need additional support.
● Continuing Education: Participates in training sessions and other learning opportunities to improve skills and knowledge related to mental health treatment, ensuring the highest quality of care and support for members.
About You
● Enrolled or recently graduated in a degree program related to mental health (eg. pre-med, psychology, social work, nursing)
● Interest in working with individuals with Severe Mental Illness and a holistic understanding of health and wellness
● Strong interpersonal and organizational skills, with the ability to adapt to changing circumstances
● Comfortable working either one-on-one or in group settings, must possess excellent interpersonal skills
● Willingness to collaborate with others
● Some experience planning and designing events and activities
● Basic computer skills and familiarity with administrative tasks, scheduling, and tracking
● Ability to work effectively with a diverse range of members and co-workers
